I, the participant, do hereby acknowledge my voluntary participation in a series of Hoop X classes and certify that my physical condition is sufficient for full participation. I understand that my participation in Hoop X involves an element of risk and a possible danger of accidents. I also understand that my participation at Hoop X may include activities involving athletes of different skill levels participating together. I assume all risks and agree to hold harmless Hoop X, its affiliated entities, and all owners, directors, coaches, staff members, volunteers, heirs, and the property owners from all claims resulting from accidents, bodily injuries, and illnesses that may arise from participation in training events and activities involving Hoop X. Knowing all these risks, I hereby assume these risks and release and discharge Hoop X and its affiliated entities from any and all liability resulting from my participation in any aspect of Hoop X.
I understand it is my responsibility to inform Hoop X staff of any medical conditions or any other special needs I might have and will notify the appropriate individuals of any health issues that might in any way affect my active or passive participation in a Hoop X class. I hereby assume responsibility for any and all costs associated with treatment for any injury or health issue that arises during my participation in a Hoop X class.
I also hereby grant permission to Hoop X to use photographs and/or video of me taken during classes in publications, news releases, online, and in other communications related to the mission of Hoop X.
